What s Melasma?
Melasma is a common skin problem. It is a common acquired dermatitis of light to dark brown macules and patches involving the sun-exposed areas of the face and neck. Most people get it on their cheeks, bridge of their nose, forehead, chin, and above their upper lip. It also can appear on other parts of the body that get lots of sun, such as the forearms and neck.
The cause of Melasma
It isn’t totally clear what causes melasma. Darker-skinned individuals are more at risk than those with fair skin. Estrogen and progesterone sensitivity are also associated with the condition. Some factors such as genetic susceptibility, ultraviolet (UV) light exposure, pregnancy, sex hormones, contraceptive pills, thyroid disease, cosmetics, phototoxic drugs (e.g., antiseizure medications) have been identified as some common contributing factors in developing melasma. Additionally, sun exposure can cause melasma because ultraviolet rays affect the cells that control pigment (melanocytes).
What is Tranexamic Acid?
Tranexamic acid is a procoagulant agent that is approved by the US Food and Drug Administration for treatment of menorrhagia and to prevent hemorrhage in patients with hemophilia undergoing tooth extractions. Through its inhibitory effects on the plasminogen activation pathway, tranexamic acid also mitigates the UV radiation–induced pigmentation response. Systemic tranexamic acid has consistently been reported as an effective treatment of melasma.
How does Tranexamic Acid effects melasma?
The intracellular release of arachidonic acid (AA), a precursor of prostanoid, and the level of alpha-melanocyte-stimulating hormone increase as the result of plasmin activity. These two substances can activate melanin synthesis. Therefore, the anti-plasmin activity of tranexamic acid is thought as the main mechanism of hypopigmentory effect of this agent. The result of a clinical trial of localized microinjection of tranexamic acid proved to be promising. Tranexamic acid exerts effects on pigmentation via its inhibitory effects on UV light–induced plasminogen activator and plasmin activity. UV radiation induces the synthesis of plasminogen activator by keratinocytes, which results in increased conversion of plasminogen to plasmin. Plasminogen activator induces tyrosinase activity, resulting in increased melanin synthesis. The presence of plasmin results in increased production of both arachidonic acid and fibroblast growth factor, which stimulate melanogenesis and neovascularization, respectively. By inhibiting plasminogen activation, tranexamic acid mitigates UV radiation–induced melanogenesis and neovascularization. There is a clinical trial proved that tranexamic acid injections has a promising result for reducing melasma. Significant decrease in the Melasma Area and Severity Index (MASI) score with no significant side effects was seen after 8 weeks of microinjection of tranexamic acid.

Melatocin Essence Dosage
- Once a week, it is recommended to have five treatments, which can be adjusted according to the patient’s skin condition.
